It's an outdoor plug in.

It looks like a pulley, which is used to carry rope or heavy cord which is passed over the pulley wheel.

Pulley assemblies are usually fixed to a wall or ceiling as a location point at the end of a line of rope or heavy cord, thus allowing a run of rope or heavy cord to be suspended between them.

It is a pulley for a specific purpose. Here's another photo.

It looks fairly industrial. The pulley wheel looks as if it is made from an undulating material. Is it used as part of an adjustable support system for overhead services cables, such as for electricity or telephones?

Wow. Spell check is so messed up it can't even spell itself properly.

Is this pulley in a garage? Used maybe for lifting engines out of cars?

It certainly is, Kevin! To be fair, I'm purposely calling it "Spell cheek" when it misbehaves. LOL.

OK. I'm going to guess that this pulley is used for moving clothing on a clothesline so you can hang it from a deck but have it high enough to catch the wind.

Kevin and Richard, you are both so close to the correct answer. It does involve laundry clotheslines. The real use is much simpler.
Here's a photo of the whole item.
