What is this accessory used for? How is it attached to the camera? What is it called?

It's an accessory for a rangefinder camera but it has nothing to do with the range finding or viewing parts of the camera.
 
The viewfinder and rangefinder are at the top of the camera. This accessory is fastened to the opposite end of the camera.
 
I can't find any pictures of it, but given your description and the looks of the device, I'd say it's some kind of fast film advance mechanism. Probably very popular with journalists in the day.

It's a trigger operated rapid-winder for Leica IIIc

If correct the win should go to twerp for the break-thru. He knew what it was by looking at it.

I favore accuracy over guesswork, so as far as I'm concerned the Admiral won. Either way, I'm happy to go with SciFan's decision.

Sorry about the absence but you are right.It is the SCNOO winder.
This particular SCNOO is the earlier version that attached to IIIa and IIIb cameras,which had a slightly smaller body than the IIIc camera.The SCNOOs for the IIIc are significantly rarer and worth several thousand dollars to a collector.
