Custom ipad forms

Does anyone know a good ipad app that allows you to create custom forms and reporting on the ipad?

I have downloaded and began a trial for Cirrus by Archon Apps and it seems to be a very handy app. I can create a custom form and save it to the cloud instantly and run very beneficial reports later. Our service techs no longer have paper to lose and I get my reports done much faster.
