1. The audio output through the headphone jack is stereo.
2. There are no special settings required to get stereo sound, it's automatic.
3. Any stereo headphone with the proper jack will give stereo sound.
4. You can use any brand of cable provided it has the proper jacks.
I think you may want to consider making a Genius Bar appointment at an Apple store. Your problem is not normal. Before going, however, try looking into the headphone jack, in case some debris is lodged there.
Make sure to do a full backup of your iPad before going to the AppleStore, as they will give you a replacement iPad if they decide your problem is a warranty issue. You can then set up the new iPad from the backup you made.
P.S.; if the audio source is mono, you will hear mono sound in the headphones.
