Safari not remembering username and password

Safari on our IPad3 is not remembering the username and password. Have gone into settings > safari > autofill > n&p and have it turned on. Went to a site and put in my username and password and got into the site. logged out and came back to the site and had to put in the info again. Toggled the names and passwords off and on a few times, rebooted the ipad both with the N&P on and off, deleted cookies and cleared settings a number of times. It seems like a setting is missing.

Any suggestions on what else to try?

Is Private Browsing turned off in Settings - Safari?

Settings - Safari - Accept Cookies should not be set up to "Never".
