You'll have to be more specific about your problem.

My ipad seems to have gone back to original factory setting. It shows my wireless connection but ipad has no information filled out in places necessary

Are you sure you didn't accidentally restore the iPad to factory settings? This is what would happen if you went to Settings>General>Reset>Erase All Content And Settings. Unless you had your iPad backed up, all your data will be lost. Your apps and other iTunes content can be re-downloaded for free by going to your purchased lists.
