Notes Problem & Sudden Return to Menu Problem (2 nonrelated issues)

Periodically, entire pages disappear from my NOTES. What causes this and how can it be prevented?

This issues occurs almost daily. I can be on Facebook or a web stite when suddenly the screen goes blank and returns to the primary menu page. Again, what causes this and how can I prevent it from happening?
