ZOOM ENABLED

My iPad 4 shows a small square transparent note saying zoom enabled at periodic intervals. Can anyone say why. I am running the latest OS

Take a look at your settings. Go to Setting - Accessibility - Zoom, and make sure it's not enabled. Also look at Accessibility Shortcut (also in General - Accessibility, at the bottom), to see that Zoom is not checkmarked.
