Unrecognized e-mail address appearing on auto-fill

In the past two weeks when a form requests an e-mail address instead of entering my email like it always has it enters one I’ve never seen before and it’s the same one every time. How can I fix this?

In a website from in Safari?

This data is pulled from your contacts entry. In Settings > Safari > AutoFill, make sure your address is selected in My Info. If it is, and the email that you are getting is also there, delete the unwanted email address.

No other ideas.
