I want to create a form for iPad use that will allow me to input answers to questions. The answers are mainly 'Yes,' 'No' and N/A. I want to be able to transfer the answers the answers to a PC where the answers for inclusion in reports. Can anyone recommend a good app. Happy to pay if it is good.

Bento might do that, at least it does the CSV export.
However I would clearly prefer iAuditor
