Unable to exit out of guided assist mode in an app even on triple clicking home button on an ipad3. Please help.

Ipad Guided Assist is not exiting

This is a known issue. You can try altering the "Home click speed" in Settings/General/Accessibility. Slowing down the required speed MAY help.
The only other way out is to reboot the iPad by holding down the home and power buttons until the Apple logo appears. This takes much longer than usual. Don't release the buttons until the logo appears.
Hopefully this issue will be addressed in an iOS update. For now, these workarounds are all I have discovered to address the problem.
