how do I get rid of this in ios 8?

In Safari, tap and hold on the icon of the site that's appearing there. A small button will show up, allowing you to delete the icon.

In Settings, clear history and website data.

Here's something else that might prevent showing Frequently Visited:

In Settings - Safari, enable "Block Pop-ups", "Do Not Track", "Fraudulent Website Warning", and select "Allow from Websites I Visit".

Then clear history and website data.

I've tried it for a while, and it worked.
