App for handwritten and typed notes on imported text

I'm looking for an app that I can take a previously prepared document file that I can import into iPad and then at my option modify the document and insert additional typing or handwriting. Eg. I want to be able to prepare a document on my office computer that is a list of questions for a court witness, import that into the iPad, then on the fly be able to insert handwritten or typed answers. I can sort of do this with iannotate, but I need the ability to modify the original document, to change the original question or increase or decrease the amount of space to insert the answers. Suggestions?
