Some cellular carriers have their own app where you can check your billing details and your data usage.
AFAIK, you can't switch a SIM card between an iPad and iPhone in the U.S. The carriers require separate plans.

It appears that the carriers are refusing to allow Facetime over 3/4G without getting a new plan.
It's not Apple. It's the carriers. Both Verizon and AT&T charge $20 to activate the hotspot.
There's lots of info on how to do it in the iPad carriers forum on this site.
