About LinkBacksI'm looking for an app to create and fill in forms. Ideally, something that will also allow me to add hand-written notes along with text fields, check-boxes and drop down lists. Any ideas?

I'd give FormConnect a try. It supports the hand written notes feature plus the various field types you are looking to create.
