When I install Skype it asks for my username and password, but I have not set these up yet. How do I do so? It will not give me the option to set up an account

You need to set up an account with Skype first, I have provided a link below for this.
Once you've done that then sign in on your iPad. Also have a look in Settings - Notifications - Skype and set up here and also Settings - Skype and activate here with notifications also.
