I frequently travel to vendor showrooms and ask for quotes on products. I need an app that allows me to take photos of product, make a few notes, and then allows me to export the photos and notes in a way that could imported into our system.

I am looking for a database app that:

* Takes photos

* Allows for quick cropping and minor editing of photos

* Has multiple, customizable fields for data entry

* Exports data to CSV or Excel that includes reference to the image file name or other mechanism for referring to the image

* Exports pictures separately from data

Any ideas?

I'd check out FormConnect. It has the ability to create customizable fields for data entry and can incorporate both photos and notes. There is also an option for exporting to CSV.
