Can somebody explain why I cannot buy an app from US iTunes Store if my iPad is register in Canada? Something's are not available on all sites? Is there a government imposed regulation or is this simply how the business is run?

The App Stores in each country, while very similar, are different. It's because of each country's rules, laws and regulations. So, yeah, you will find stuff on the Canadian store that's not on the U.S. store - and vice versa.

If I buy an iTunes card in the US, will it work with my account or is it based on the location when I try to redeem it?

Best way is create a new us account with an American address. I suggest also a new gmail address that way it is separate. Do not say credit cards when setting up account after new account is accepted then redeem the us iTunes card in the us account. Hope that makes sense
