Lacking a tripod, you could try Camera Awesome! instead of the standard camera app. It has an image stabilization function that might help a bit. But your main problem is noise, and to fix that you'll simply need more light. What you want is to able to adjust the ISO setting lower, and make a time exposure. But AFAIK you cannot do that.

You can try Pro Camera HD for the Ipad( don't confuse it with the iphone one) It takes better pictures than the standard camera app.
If the problem is too little light, long exposure, then you need a tripod.
If the focus is off, be sure to tap on the object of interest to get the camera to focus/expose on it.
